TO SARAH/TX: BYETTA/METFORMIN

I can’t answer all, and I have no clue abt insulin, I am only on Byetta 10
(recently switched from 5), BUT I CAN talk about the Metformin!
I had alot of diahreah with it, then she switched me recently to AVANDAMET…
she said it is easier on the stomach. It has something else in it with the
metformin. (There ARE other types of ‘metformin’ so if the one you are on
bothers you too much, ask abt alternatives.)
Why do you want your A1c to be in the "5’s", when it’s great to have it at 6.5
or less?
PS I had alot of nausea, ALOT, when switching from Byetta 5 to 10, but I found
that as long as I eat a good sized meal, not just a little br’fast of say, one
piece of toast, after abt a week, the bad nausea subsides and it’s do-able. But
I cannot handle it if I am not eating a ‘real meal’ … and I’m not a morning
br’fast eater, so I do my first Byetta for ‘brunch’ or lunch. Making sure I have
5 hrs in btwn that and my dinner injection.

And I have had "heart" issues for a while, long before going on Byetta, so I
corelate anything to the drug. A little over two years ago I went to the ER, and
was admitted to the hospital for chest pains of unknown origin. They determined
it wasn’t a heart attack. I have had some pains since, but not the same.
Yesterday I went to my cardiologist for my annual check up. When I told him of
the pains (I guess I forgot about the previous pains, if it didn’t happen
yesterday or I don’t write it down, I forget) and now I am scheduled for some
catscan like test next month. He’s hoping to narrow down the source of the
problem. And for the test I have to stop taking Metformin for a bit before the
test and 48 hours afterwards. I love the concept of a noninvasive test that can
make my body explode. :)

Off Byetta for two days now and itching is much less. May be a
coincidence, but I’m staying off it until I see doctor.

I’m on the 5 unit dose of Byetta, and it works GREAT! actually,
sometimes a little too well. Sometimes 2-3 hours after a meal, I’ll
get a low of 62 or so. Which is very frustrating because if the meal
was dinner, then I eat a snack, and my BG goes up at bedtime. So THEN
my liver dumps glucose in the night and I wake up at 140. Does anyone
else have this problem? Suggestions? My liver dumps about 40 points
worth of glucose EVERY NIGHT. It’s not triggered by a low (I know,
because a couple of nights ago I tested every hour on the hour ALL
NIGHT, and it was fine until it started ramping up), so what I really
would like to be able to do is be at about 80 when I go to sleep,
that way I’m 120 when I wake up. This is the plan anyways, I don’t
know if it’ll work or not, but I want to try it.

My endo is happy with my numbers (diagnosed at 8.4, then 7.6, now 6.7
as of Tuesday of last week), and says as long as my morning numbers
stay at or below 140 she’s happy. Actually, we tried insulin
(levimere???? something like that) and at 10 units we were just
staying the same. My liver dumped extra glucose to compensate for the
extra insulin. I am loosing 5-10 lbs a month on Byetta, so we really
don’t want to up the insulin too much, because she worries that it
will stop the weight loss, or slow it down. Actually, if I’m
perfectly honest, I’m the one that worries about that, and she just
goes along with me.

The thing is, I’M not happy with my morning BG levels. I don’t like
these large swings (in the morning I can go from 140 to 62 in 2 1/2
hours). She wants me to test some more, give her some more
information, write down EVERYTHING I eat…etc… She also thinks I
am shooting for too much when I tell her I want my A1c to be in the 5
range. She worries I would have too many lows. Thoughts?

I know that metformin would help with all of this, but my stomach
can’t take it (I prefer to be a little sea sick than the
alternative). I have heard that different fomulations of metformin
(extended release, other kinds) affect people differently, can anyone
talk about that?

Thanks for reading all of those questions. I’ve kindof been storing
them up.
